IMPORTANT!  Do not place a "tax-exempt" order on this website until after you have submitted this form and received confirmation from us that your account has been approved and set up for tax-exempt status.  ELEY does not refund sales tax paid on orders that were placed before the sales tax-exempt status was established.

Use this form to request a sales tax-exempt account with Eley Corporation. Upon approval of your tax-exempt status (usually on the same or next business day), you'll receive notification verifying your new tax-exempt status.  You will then be able to place your tax-exempt orders on this website. 

NOTE:  You'll need to upload either a PDF or a photo image of your sales tax-exempt certificate at the bottom of the form.
